How to Get Started Initial Assessment: After your accident, you typically need to visit a physiotherapy clinic first for an initial assessment. Treatment Approval: Once assessed, you may be approved for up to 10 or 21 treatment sessions depending on your diagnosis. Required Forms: Before your first session with us, please complete our MVA Intake Form and Initial Assessment Form. Documentation: Please forward your treatment approval email from your insurance provider to: ? lunaswellness.mva@gmail.com Booking: Once we receive and verify your approval information, you can proceed to book your massage or acupuncture appointments. 2. Direct Billing & Financial Responsibility (Please Read) To make your recovery as seamless as possible, Lunas Wellness offers Direct Billing for approved MVA treatments. By seeking treatment with us, you acknowledge and agree to the following: No Guarantee of Payment: Direct billing is provided strictly as a courtesy. It does not guarantee that your insurance company will cover the full cost of every session. Personal Responsibility: If your insurance company denies a claim, delays payment, or fails to pay the full amount for any reason, you are personally and fully responsible for any outstanding balances. Credit Card on File (Required): To ensure seamless processing of any balances not covered by insurance, providing a valid credit card on file is mandatory for all MVA patients. Your information is stored securely in compliance with privacy regulations. Cancellation Policy: Cancellations or rescheduling within 24 hours of your appointment will incur a fee equal to 50% of the session cost. No-shows will be charged 100% of the session cost. Please note that insurance companies (MVA, WCB, or private) do not reimburse these fees; you are solely responsible for these charges.
